For over 110 years, the Leavitt Family Jewish Home has been dedicated to offering exceptional nursing care services, delivered with skill, compassion and respect. People of all faiths put their trust in us.

Our mission is to provide personalized care in a “home-like” setting. Situated on a 23-acre campus of care, nestled against woodlands in the town of Longmeadow, people fondly refer to us as “The Home.”

CARE OPTIONS

Long-Term Care

Long-term care is needed when the care needs of an individual have exceeded what can be provided in the home or in any other setting such as assisted living. Units are focused on an individual’s care needs. Activities are designed to assist residents and meet them where they are.

Short-Term Rehab

For those times when you need additional therapy or medication management post hospital stay with the goal of returning home. Short-term rehab gives you the confidence to return to life as you know it, despite post-surgical fatigue or a new deficit. Physical, occupational, speech, cardiac, orthopedic, and pulmonary therapy can all be part of a short-term rehab plan.
