For more than a century the Leavitt Family Jewish Home has been dedicated to offering exceptional nursing care delivered with skill, compassion and respect through the dedicated efforts of our specially trained staff members. This is why people of all faiths put their trust in us.

Our mission is to provide personalized care in a “home-like’ setting. Situated on a 23-acre campus of care nestled against woodlands in the town of Longmeadow, people fondly refer to us as “the Home”.

Skilled Nursing & Long-Term Rehabilitation

We provide 24/7 long-term skilled nursing care, as well as full service rehabilitation therapies, palliative and hospice care, and certified music therapy. We offer personal care and assistance with activities of daily living. Medication management as well as access to an on-site physician, nurse practitioner, dietitian, and social workers is provided.

We admit individuals who may need complex medical/surgical services, IV therapy, comprehensive medical rehabilitation and respiratory therapy, and patients who have been hospitalized with neurological and orthopedic conditions. People requiring short-term rehabilitation have access to our Sosin Center for Rehabilitation.

Accommodations

200 bed facility with 112 private rooms, divided into five neighborhoods. Our rooms feature bathrooms, flat screen TV’s, cable television; wifi access; telephone service, large windows, and scenic views of the beautiful landscaped grounds. Each neighborhood offers a cozy fireplace living room, inviting dining space, and sitting areas. Each ground level neighborhood offers a lovely courtyard.

Amenities

In addition to caring for our residents’ physical needs, we understand the importance of meaningful stimulation and engagement. Amenities include: beauty salon; coffee and gift shop; banking services; nutritious, kosher meals; Michael’s Café – offering light Kosher meals and snacks; inviting dining, social and recreation areas; outdoor spaces with benches, walkways and gardens; planned recreational activities and special events; and spiritual support, including Rabbi on-site and interfaith clergy.

Rehabilitation Services

Our dedicated and compassionate staff bring extensive experience in their fields and we have access to a wide range of medical experts. Therapies include physical, occupational, and speech therapy. Specialized therapies include cardiac rehabilitation, orthopedic conditions, pulmonary rehabilitation, heart failure, wound care, neurological conditions, and complex medical conditions. Medication management as well as access to an on-site physician and nurse practitioner is provided.

Customized Care Plans

Our multidisciplinary team develops an individualized and comprehensive plan of care for every resident. Our skilled staff provides daily assistance with all personal care needs and activities of daily living. A core philosophy is that residents are a part of their own treatment planning. This optimizes recovery and helps reestablish independence. Residents’ families are part of all comprehensive care plans, receiving support and counseling as needed to ensure a smooth adjustment to care.

Activities

Men’s and women’s group, current event discussions, Veteran’s gatherings, movies, cookouts, pet visitations, a wide array of entertainment, concerts, art projects, music programs, poetry reading, religious services, bible study, bingo and holiday celebrations.

Morning Schedule

Typical morning consists of an arts program such as flower arranging, song writing listening to music, baking or a creative group discussion.

Afternoons

Afternoons are filled with guest entertainers and presenters, competitions involving staff and residents such as bowling and spelling bees, birthday/holiday celebrations, and group activities and discussions.

Evenings

Night activities include card games, art projects, concerts, talent shows, one-on-one staff visits.
