Spectrum Home Health & Hospice Care, a member organization of the JGS Lifecare system of care, joins the Visiting Nurse Associations of New England Network
Spectrum Home Health & Hospice Care, part of JGS Lifecare in Longmeadow, provides in-home skilled nursing and rehabilitation, home health aides, palliative care and hospice services throughout Hampden County. It recently became a member of the Visiting Nurse Associations of New England (VNANE), an umbrella organization that provides a range of administrative and support services for its network members.
VNANE is the largest provider network of its kind in New England and a national and regional leader in patient care, quality improvement and clinical education. Its members are nonprofit, community-based home health care and hospice agencies operating in Connecticut, Massachusetts and Rhode Island. VNANE’s clinical pathways and clinical education programs have served as a model for the home care industry serving as the basis for the Visiting Nurse Associations of America (VNAA) Blueprint for Excellence program.
“Our membership in VNANE will enhance our existing home health care and hospice services, as well as benefit our patients,” said Madeline Presz, executive director of Spectrum Home Health & Hospice Care at JGS Lifecare. “Patients will experience increased access to services, most notably preventative health maintenance care, which has been proven to reduce hospitalizations and prevent or slow disease progression.”
As a result of VNANE’s standardization of care, an Integrated Care Management Program for Health Plans and ACOs (accountable care organizations) has been implemented. This coordination of care program has an interactive patient education and care management component. It has proven to reduce emergency room visits and re-hospitalizations. It promotes healthy outcomes and reduces costs of care through VNANE’s rapid centralization of services with a consistent coordination of care and commitment to results reporting.
