Home Health & Hospice Care
Spectrum Home Health & Hospice Care provides quality care that comes to you, in the privacy and comfort of your own home. We coordinate medical care with community physicians, families and multiple service agencies. Services, including wound care, medication education, medication monitoring, and therapy, are delivered throughout Hampden County, serving homes in Longmeadow, East Longmeadow, Springfield, West Springfield, Agawam, Chicopee, Hampden, Ludlow, Westfield and Wilbraham. Patients are referred to our home health and hospice services by acute care hospitals, physician offices, and skilled nursing facilities, as well as personal choices and recommendations.
Home Health Services
At Spectrum, we pride ourselves on conveniently bringing a wide variety of healthcare services into your home. We provide occupational therapy for seniors who have had injuries or strokes who need help with the activities of daily living, such as cooking, bathing, or dressing. We also coach recovering seniors through potential environmental adjustments. Speech therapy is also available for patients with speech, memory or swallowing issues. Our occupational and speech therapy services can help your loved ones if they are suffering from joint replacements, neurologic disorders, arthritis, traumatic brain injury, or other debilitations of motor skills.
Hospice services, including symptom management, social work, pastoral care, palliative care, and home care, are available to patients who have a prognosis of six months or less to live. Trained therapy dogs and a therapeutic harpist are also available to help people focus on a better quality of end-of-life to transition gracefully. Our hospice services include helping not only your loved one deal with the physical, emotional, and spiritual aspects of their situation, but also helping you and your family coordinate health care and assist in goal planning.
Hospice and Home Health Accreditations
Both Hospice and Home Care programs have been recognized with accreditation by the Joint Commission. Both are located in the Nirenberg Administration Building on the Harry and Jeanette Weinberg campus in Longmeadow. Services are covered by Medicare and Medicaid, as well as most other major insurances.
