Spectrum Home Health & Hospice Care provides quality care that comes to you, in the privacy and comfort of your own home. We coordinate medical care with community physicians, families and multiple service agencies, and provide a personalized care plan for every patient.

If you need help recovering from an illness, injury or surgery, our Home Health Services can provide the expert care you need to recover in the comfort of your home.

When a cure is no longer the goal, our Hospice Services allow patients with a life-limiting illness and their families to take a proactive step to ensure that needs and wishes are embraced, and that all the care possible will be given to manage symptoms and optimize quality of life.

When you need help managing the symptoms and side-effects of a chronic condition or serious illness, our Palliative Care Services offers patients relief and support, and the best possible quality of life.

We provide Spectrum Home Health & Hospice Care throughout Greater Western Massachusetts, and our team includes Spanish-speaking care providers. We can accept patients within one hour of new service requests, and are able to start services within 24 hours.

We care for patients with a broad range of conditions, including injuries; strokes; speech, memory or swallowing issues; joint replacements; neurological disorders; arthritis, traumatic brain injuries; or other debilitating conditions. Patients needing more intensive rehabilitation have access to our Sosin Center for Rehabilitation for short term assistance, or our Leavitt Family Jewish Home for long-term rehabilitation and care.

Hospice Services

We offer hospice services for patients who have a prognosis of six months or less to live. Our hospice services include helping not only your loved one deal with the physical, emotional, and spiritual aspects of their situation, but also helping you and your family coordinate health care and assist in goal planning. Services include:

Palliative Care Services

JGS Lifecare’s Frances B. Cohen Palliative Care Program provides compassionate, patient-centered care to people suffering from a wide range of chronic and complex conditions.

Palliative care is a specialized branch of medicine that uses a team of medical professionals to help patients better manage the symptoms and side-effects of a wide range of chronic conditions like Alzheimer’s, congestive heart failure or cancer, among others, while also addressing emotional and spiritual needs.  The team works with the patient’s family and the primary care physician to provide that extra layer of skilled health care and support, helping families coordinate care by multiple providers, and assist in goal setting and care planning.

While sometimes used in conjunction with Hospice Care, patients do not need to be at end of life to receive palliative care as it addresses symptoms of illness and side effects of treatment and is intended to be supportive care for seriously ill patients who have special physical, emotional, and spiritual needs.  In fact, palliative care is often used to help patients manage the often deleterious side effects of some curative treatments, like chemotherapy or following surgical procedures, helping patients address symptoms and medication management.

Both our home health care and hospice programs are accredited by the Joint Commission.

Payment

Medicare and Medicaid, as well as most other major insurances, may cover home health care, hospice and palliative care. Call our office to determine if your insurance plan includes a home care, hospice or palliative care benefit.
