On September 11, 2018, notification of a proposed change in ownership of Jewish Nursing Home of Western Mass., Inc. (“Applicant”), 770 Converse Street, Longmeadow, MA 01106 was submitted to the Department of Public Health, Division of Health Care Facility Licensure and Certification. The Applicant, if deemed suitable by the Department, intends to continue operations of the nursing home subject to approval of changes in its corporate structure with Chelsea Jewish Lifecare, Inc. becoming the sole member of the Applicant’s sole corporate member, JGS Lifecare Corporation. There will be no changes in the bed capacity or the services provided. A public hearing may be requested upon petition by any group of ten adults. Such petition shall include the name, address and signature of each adult and designate one member as the representative of the group. Written comments concerning the Applicant’s ability to provide quality long-term care services and petitions for a public hearing may be addressed to the offices of the Department of Public Health, Division of Health Care Facility Licensure and Certification, Suitability Review Office, 99 Chauncy Street, 11th Floor, Boston, MA 02111, or by email to HFLLicenseAction@state.ma.us, for a period of 14 days following this publication.
