Project Transformation

Project Transformation is underway to enhance, build and expand our services and facilities to improve health outcomes, and enhance resident dignity, independence and quality of life.

With the opening in December 2018 of Sosin Center for Rehabilitation, our state-of-the-art short-term rehabilitation center offering 24 private rooms in the small house model of care known as the Green House model, and the simultaneous opening of Michael’s café, Phase I of Project Transformation
has been completed.

Project Transformation will now embark on Phase II fundraising, bringing the relationship-based small house model of care to our long-term care residents at the Leavitt Family Jewish Home, as well as other significant updates to the entire campus.
