Registered Nurse/Supervisor-Part Time

Supervisor/RN

16 hours/week
Shift: 3pm-11pm and every other weekend

POSITION SUMMARY: The primary purpose of this position is to impart leadership, supervision and support to staff on the 3p-11p shift with the goal of providing resident care in accordance with the philosophies, objectives and established policies of the facility and the nursing department; and to act as a role model for others on your shift in your daily interactions with members of the interdisciplinary care team.

DELEGATION OF AUTHORITY: As 3p-11p Supervisor, you are delegated the administrative authority, responsibility and accountability necessary for carrying out your assigned duties.

DUTIES AND RESPONSIBILITIES:

 
REPORTING/COMMUNICATING FUNCTIONS:
  1. Receives verbal report on residents noted in the communication book from previous shift and reports all significant resident changes to the oncoming shift at the end of the day
  2. Makes rounds at beginning and end of each shift and during the shift as needed
  3. Reviews residents status and communicates pertinent information to others on the team to assure that all residents are accounted and cared for
  4. Notifies DON and/or ADON as soon as practical of changes in condition, transfers to hospitals, incidents and accidents, abnormal lab values and any other follow up issues/concerns
  5. Evaluates residents conditions and consults the physician when necessary
  6. Follows through on all physician orders
  7. Meets with 11p-7a Clinical Coordinators, gives report and ensures that his/her staff have completed their work. Assists them as needed to prevent O.T.
  8. Arranges hospital transfers as needed
  9. Informs necessary disciplines in the event of accident, injury, change in condition, transfer or death of resident in his/her care

SUPERVISION FUNCTIONS:
1.   Coordinates CNA’s meal and break times and unit responsibilities
2.   Monitors CNA performance and provides situational teaching prn
3.   Supervises the work performance of nursing personnel and assists as needed to insure orderliness of the wing and that all aspects of the resident’s care plan are carried out properly and in a timely fashion, including restorative care and all aspects of ADL’s
4.   Insures restraints are used properly and only when indicated
5.   Holds staff accountable for performing and completing tasks
6.   Provides resident teaching and support.  Assures that CNA’s are part of the supportive environment
7.   Notifies nursing office of special supply/equipment needs for shift
8.   Assumes additional duties as assigned by the DON or ADON

IMPLEMENTATION OF RESIDENT CARE FUNCTIONS:
1.   Assumes responsibility for care to all residents on the shift
2.   Sets and adjusts priorities in the provision of resident care
3.   Evaluates appropriateness of resident care plans
4.   Evaluates resident care skills of staff on his/her shift
5.   Recognizes own limitations and seeks assistance from administrative resources as needed
6.   Monitors weekly weights and assesses needs of residents and follows dietician’s recommendations relative to nutritional needs of all residents
7.   Monitors weekly skin surveillance and assesses appropriate RX’s
8.   Participates in transcription and noting of physician orders as needed
9.   Participates in decubitus reports and weekly skin surveillance
10.  Participate in comprehensive assessments
11. Organizes and assist licensed staff in ensuring that all lab work is done according to M.D. order
 
DOCUMENTATION AND COMMUNICATION FUNCTIONS:
1.   Documents resident responses to RX’s, procedures and meds as needed
2.   Communicates clearly, concisely and accurately in shift to shift report
3.   Records and reports resident information through appropriate channels
4.   Demonstrates courtesy, tack and professional behavior in all interactions and relationships with residents, families, visitors, ancillary staff and others
5.   Demonstrates awareness and support of resident rights and reinforces this in others
6.   Completes assessments and care plans as necessary and oversees that others do as well.  Assures that monthly edit process is followed.

MANAGEMENT AND LEADERSHIP RESPONSIBILITIES:
1.   Demonstrates leadership skills for 24 hour accountability
2.   Provides guidance and direction to licensed staff and CNA’s
3.   Collaborates with nursing administration in unit management regarding staff and personnel
      and policy issues
4.   Determines appropriate assignments and adjusts same to meet resident needs
5.   Demonstrates compliance with regulatory codes, nursing policies and professional standards including medical/legal concepts
6.   Acts as a role model with demonstrated knowledge and proficiency in the management of
      resident care, directs new staff as appropriate
7.   Develops and expands own nursing skills as well as those of others
8.   Identifies and strives towards professional goals
9.   Assumes other responsibilities as required by the changing and expanding nature of the
      nursing department
10. Identifies educational needs of staff and communicates same to Director of Staff Development
11. Replaces all applicable 3p-11p or 11p-7a sick calls
12. Is available to assist on any unit during resident emergencies, treatments, shift issues, resident rounds or any other issue needing additional licensed staff support
13. Assumes wing specific change responsibility
14. Participates in their own assigned Q.A. and ensures follow through on resolutions for all deficit areas
15. Assures that monthly edit process is followed

PROFESSIONAL CONDUCT RESPONSIBILITIES:
1.   Is a role model to others as demonstrated by courteous, supportive interactions with all other nursing team members
2.   Interacts positively with other departments to promote harmony within the facility
3.   Attends and encourages staff to be present at in-service education programs presented both within and outside the facility

8.   Abides by all facility policies and procedures including but not limited to personnel/Human
      Resource policies, safety and security and all nursing department policies

 

QUALIFICATIONS:

  • Graduate from accredited school of nursing
  • Current Massachusetts licensure and valid up to date CPR certification
  • Current clinical practice with minimum of two years experience
  • Evidence of progressive responsibility in management capacity
  • Ability to adjust to new or changing situations and stresses
  • Possesses a high degree of initiative, resourcefulness and maturity of judgment
  • Possesses strong interpersonal skills, leadership ability and excellent communication skills
  • Ability to stand, sit, walk for prolonged periods of time on a frequent basis
  • Ability to bend and squat for prolonged periods of time on an infrequent basis
  • Ability to push, pull objects and lift/carry up to 20 pounds unassisted on a frequent basis and up to 50 pounds or more with assistance on an infrequent basis
  • Ability to interact effectively with residents, their families and other staff members
